ACCEPTING APPLICATIONS FOR BOTH SESSIONS!!

We have three ways for you to register: Online, by US Mail, or by FAX. To register by US Mail or FAX you will need to download the Application/Brochure below. The Application/Brochure is in Adobe Acrobat format and you will need Adobe Acrobat Reader to open and print the file. (Acrobat Reader can be downloaded here.)

ONLINE REGISTRATION: We have teamed up with Active.com to bring you online registration. If applying online, you do NOT need to mail or FAX an application to us. There is a $12 surcharge for online registration added to the tuition charge. (please note the online registration will open in a new window, and if you have any pop-up blockers, please disable them temporarily):

If applying via Fax, Fax the completed application with your credit card information to 812-824-7747. When faxing the application the full tuition fee will be charged. No Deposits accepted when using a credit card, full payment only.

If applying via US Mail, mail the Application with payment to the address at the bottom of this page.

Please Note: The Girls Indiana Soccer Camp is a separate camp, you will need to contact them directly for registration as we do NOT accept applications for the Girls Indiana Soccer Camp
